Thank you for your interest in joining CSYO! 

 

HOW TO REGISTER FOR THE 2016/17 SEASON:

1. Visit the CSYO Blog, csyo.wordpress.com and click on ‘Downloads’
2. Download the registration package and fill it out.
3. Submit the form electronically to CSYO, using one of the two methods below:

a. Click on the ‘Submit’ Button

b. Save the form to desktop and e-mail to cs.youthorchestra@gmail.com 4. Print a copy of the form out and sign it.

5. Hand in payment with the signed paper form to CSYO Administration by:

a. Drop it off “the registration box” at 47 Proctor Avenue, Thornhill, ON.

b. Mail to the attention of “CSYO Administration” at

47 Proctor Avenue,

Thornhill, ON L3T1M6

 

Note: Registration is NOT complete unless a PAPER COPY AND an ELECTRONIC COPY of the form is received with full payment.
